I attempted to create a new outpost, but the button was faded out and hovering the mouse over the button brought up the message that my "settler" was too close to another city. However there was not a city near by.

did you try geting the settler OUT of the stack, then try creating the city.
it could be related to the stack bugs that are claimed to be fixed in 3a
