I don't think it's intended that the map should be generated that way, unless the developers changed it from FE. At least I remember reporting a similar problem in the FE beta and getting a response that it was fixed.
It was times like this that made the old control-U cheat (to reveal the whole map) valuable. Try going to your Program Files\Steam\SteamApps\common\FE Legendary Heroes directory, find LegendaryHeroes.exe, and send it to the desktop to make a shortcut. I also defeated Magnar, but the "Exterminator" "defeat a faction" achievement is still locked, as is the "Defeated Sovereign in Battle" achievement.
I cleared the wasteland and defeated the Pyre of Man, but the Steam achievement is still showing as locked.
I had originally equipped it on General Carrodus when he had finished what I thought was the last part of the quest (the 3rd battle), and had then later traded and equipped it on his henchman. I've tried unequipping it from both, and the quest still won't finish.

No, never lost my first city. I only noticed this after capturing a couple of Magnar's cities and razing them. I can't pinpoint exactly when it started though.
It's also letting me build a second Adventurer's Guild and a second Forge of the Overlord.

I already have a Tower of Dominion in my first city: Now, many many turns later, another city I had founded (not captured) is now able to build another tower of dominion, despite the limit on one per faction: Saved game is here, I hope it helps.
I just did "The Assassin's Blade" quest for the first time on LH, and despite killing all 3 targets of the assassin, the quest won't clear. The last stage says "Have the Assassin's Blade". I have the blade, and have equipped it on the champion that did the quest. Does it have to be equipped on the sovereign to clear the quest? Is this a bug?
Go to the game options and turn off anti-aliasing. It might not work, but it has helped others.

When the borders first expand (or an outpost is built) to make the connection, it always seems to wait until the next turn to recalculate the unrest. I believe I've seen it take 2 turns to do so on occasion also. I haven't ever noticed it reverting, I'll start looking for that.
I did the quest that gave me the Panca archers. Since I already had a full army of troops, the Panca archers stacked with my army. When I selected the archers and moved them away, my army stack was shown on the map as only my sovereign with no other troops: As you can see from the bottom part of the screen, she's not alone. I had a troop of custom horsemen traveling along this road, and when it was their turn to move at end of turn, they started at the beginning of the red arrow and traveled straight along the arrow's path, right through the water. I don't know if this is something that needs to be fixed, but it looked so funny I had to share it.
This just happened to me too. Altar, who I have friendly relations with, asked me to end our (non-existent) war. According to the foreign relations screen he's not at war with anyone else either.
After a battle I received the "Flickering Ring" which says it allows the wearer to cast Blink. Blink is not listed as a spell in the Hiergamenon:

I don't know if it's supported or not, but since Steam stores the saved games in the cloud I thought I'd try starting a game on one machine and continuing it on another. Both machines had .75 installed. The game was started on a Vista 32 bit system and then saved after a few turns. Shutdown the machine, booted my other one (my main gaming machine), a Win7 64 bit system. Loaded the saved game, it appeared to load ok. When I hit the turn button
