I have a Humble Inn with a medium difficulty quest that doesn't activate when a champion enters the tile. I've tried various combinations of champions but nothing works.
I just completed the humble in quest, no problems. Is there only one humble in quest?
You should post you save. If you need a place to host the save file I suggest you create a Dropbox account, you can post the file there for the public to download.
I've seen multiple humble inns within ten tiles of one another in a game. One of them listed its quest as weak, the other as medium.
I suspect there was just a cheap architect at one time hitting up innkeepers with pre-designed elements for matching floorplans.
If you have a quest already open (like from another humble inn), then this quest may be the same one and the intended effect is for there to be no quest active. But if you are clear of quests and this happens, then it is definitely a bug and you should post or send in your save game to support to help them find and fix the problem.

I've had this happen tons of times. It seems to particularly happen in more advanced games. You can really experience the bug well by purchasing lots of quest scrolls. Some of them work, some don't; however this issue is not limited to quests generated by the quest scroll.
I had exactly the same thing happen with a weak difficulty Humble Inn quest. I sent Kael the save file and he emailed me yesterday to say that it was fixed.
