My sovereign found a pair of patchwork greaves, so I went to the items menu and double clicked on them to equip them. The icon changed to show that they were equipped, and I could see that his defense had increased, but before I could do anything else the game crashed.

I attempted to load the last autosave from the above crash, and it puts up the "Loading worldname" window and then crashes again.

Same thing happened to me.
Also it takes about 10 tries to load a game.
