Debug.err files are always generated on every run, regardless what happens. See the reporting guide sticky for details.

As publius said, we need screenshots showing us what you're seeing, as well as your debug.err file. The reporting guide he linked will describe how to provide these.
Do you have an autosave that allows this to be reproduced? If so, we can use it.
Windowed mode or fullscreen? Taskbar set to show or autohide? Always on top or not? Please provide more detail about this issue.
