I don't know if this has been mentioned before, but I've found that if you rename a NPC, the ones you can hire and are running around the map, it doesn't change their name in the description about them. Also it seems that there are a couple of instances of Kingdom names doing the same thing. When I got married it said that everyone in the Kingdom of Reland, or something like that, not Ryleth, the name of my Kingdom. Don't know if thats a lore thing based on faction deciding your overa

Try going to your options menu and messing with the "Tactical Threshold" slider. It decides what the number of enemies you need to do a tactical battle. If you want it to always ask you if you want a Tactical battle or not set it to 0. Hope that fixes it for you.
