Yes it was Kraxis. I'm guessing you are right and they are using Broken Loyalties. It would be nice to have a message explaing what happened. I figured it was a bug.

[0.92] Scenario - Lost city without battle in LH Support

In the scenario I keep losing a city that only has about 20% unrest. It says the city was conquered. Any units I have stationed in the city are moved outside its walls.
