Honestly as long as "custom" remembers all your choices you pretty much have Play Now. I'm the type that goes out of his way to set everything up the way he likes it and then uses the same options most of the time. Likewise in this game if I have my custom civilizations set up and a map type that I like I'll be happy to hit "next, next, next... game on!"
