Given the relatively low number of items, I'm dubious whether there is a bug. This is exactly the same as the itunes shuffle "problem", and Apple came up with various solutions, including (for example) "without repetitions". "Without repetitions" is not random at all, but it may be more what people expect. So two solutions spring to mind for legendary heroes: 1: add more items; 2: add a world option for "no repetitions" for loot drops.

Probability is confusing. If I have three dice, the chances of rolling a 1, a 1, and then a 2 are 1/216. But if I don't care about the order as long as I get a 1, a 1 and a 2 (so in other words 112 and 121 and 211 are also valid) then the chances treble to 3/216. If there are 19 Uncommon armour drops and I have two drops, what are the chances both will be Assassin's Vambraces? 1/361. But if I have three drops, what are the chances at least two will be Assassin's Vambraces?

I assume the faction you're trading with likes Technology treaties. In some circumstances I believe they will pay you to have a technology treaty.
I did get a lot of Assassin's Vambraces in my last game, I mean like three or four of the darned things in the first 100 turns. Random number generator issues are notoriously hard to prove though, people forget that rolling 1, 1, 1 on three dice is not impossible, just unlikely. 1/216 is not even close to "never". I've noticed that zero seems to be negative for treaties. From memory you can see the same thing if you give gold to bring the value from negative up to zero. At zero the AI says No, whereas for other, non-treaty trades zero is ok. I assume for treaties the test is for > 0, for other trades the test is >= 0. Probably not intended, not exactly a massive bug but probably quite easy to fix.
I do build Studies in my Fortresses, but most of the time I build troop enhancing buildings first, and if I need units, I'll be training them rather than building Studies. (The big exception is if my capital is a Fortress, +1 Research at the start of the game is actually quite a lot.) I get the impression that the AI doesn't really specialise its cities like a good human player does. Certainly I've captured large AI cities late in the game which had built almost everything

As I understand it each of the factions has a personality which prefers building some buildings to others. I haven't looked at how this works in practice but I assume simplistically that some factions prioritise expansion, some prioritise military units, some prioritise research, etc. This does put the AI at something of a disadvantage against a player who prioritises production maximising and unrest reducing buildings, but it is a valid design decision to not make all AIs play the same.<
Really interesting discussion. Other than Haste I don't cast buffs very often, but that's mainly because I tend to play with Death/Air/Water mages. Life has Growth, Fire has Focus (incredibly cheap and +25% damage is probably worth using a turn). It's Earth who has the best buffs though. I agree the trade-off is between mana cost and other costs, particularly healing units or worse, having a high level unit die. In an average late game battle I might cast only Haste at a cost of 2
My first thought when I read this post was Call of the Titans, which could instantly bring together a lot of enemy armies in your territory. To the best of my knowledge I've not seen the AI cast Call of the Titans, but on Insane, who knows! I would expect it to have researched the tech by turn 100 on Insane. I've had this problem (lack of iron) in maybe one game I've played (ignoring a couple of games where I died very quickly). Provided you expand effectively at the start it's very unusual to be unable to get iron. I have built Infirmaries a couple of times to aid healing of troops, notably when I was playing with Juggernauts and they kept getting splash damage. I have never chosen +2 metal on a city level up. You need maybe three blockers, but as long as you have troops who you can put on defend while your archers take the enemy down it should be fine. Make troops with leather armour, wooden shields, swords, Ironskin, Defender, Constitution, you should do fine. Summoned troops are also a possibility. Lightbringers, skeleton horde, Cragspawn (cause stinking mud for one tile radius), Earth Elementals, all make good blockers.

Hmm, I suspect they may put that down under "exploits which can be fixed by not doing that"... having said that, maybe there's an easy fix.
