[0.86][Bug] Quest Hero's Tomb doesn't give the 'magical item' reward

After defeating the dragon the pop-up says 'you find some gold and a magical item', however I didn't receive an item, unless the plate item I gained at the start of the quest was it.

I think I got a toxic knife out of it, my third one that game.

I've repeated this quest and same result, gold but no item. I've also a save game just prior to fighting the battle if needed to verify. I'll try and reproduce this in another game as well :)
