A couple times now, I've gotten into a fight right after getting a quest, and lost the fight so I loaded the autosave. It had autosaved me ontop of the quest building, but the Quest items had not spawned.  First time this occured was it was the "Escort Daughter" quest, and the second time it occured was with a Kill the Bandits quest, for a Refuge Camp.  Both times I loaded shortly after getting the quest, to not be able to trigger the quest again.
